VITALITY UNLIMITED HAS EXPANDED OUR SERVICES!
With this addition to our CCBHC Program, the “One-Stop-Shop” model is greatly enhanced. Our 10 Core Services of a CCBHC includes Primary Care treatment.
Some of the additional services that we can now offer include:
- Accepting new clients of all ages for acute and chronic health conditions
- Kids 0-18 walk-in
- Full spectrum of family medicine
- Primary care screening and monitoring of key health indicators/health risk
- Examine patients and establish medical diagnoses by client history, physical exam, and other criteria
- Admit patients to and discharge them from healthcare facilities
- Order, perform, and interpret lab, radiographic, and other diagnostic tests
- Identify, develop, implement, and evaluate a plan of care
- Perform therapeutic procedures
- Prescribe treatments and medications
- Physicals for work and school DOT/CDL exams
- Referrals for specialty consultations
