Homeless Veterans Services

Are you a Homeless Veteran or do you know one? Do you need help transitioning from homelessness to housed? We can help.
Vitality Veterans Program works with the VA Sierra Nevada Health Care System, Healthcare for Homeless Veterans (HCHV), to offer a transitional housing program for Homeless Veterans. The goal of our program is to move you or the Homeless Veteran into independent housing as quickly as possible.

The Vitality Veterans Program is on a 2-home campus in Sun Valley, Nevada. This facility provides:

• Referral counseling services for mental health and substance use disorder treatment
• Targeted case management
• A clean, safe environment
• Beds and linens
• Snacks and professionally prepared meals
• Onsite laundry facilities
• Public transportation access
• Connection to VA and other medical services

Sage House Transitional Living for Men

Sage House provides professional support in a stable, alcohol, drug, and tobacco free environment at an affordable rent. Males 18+ are eligible for admission. The first 30 days of residency may be covered by Nevada Medicaid for eligible residents. Thereafter, the resident is required to stay current on their rent. Rent includes some food, telephone, TV, internet, other utilities, and laundry facilities. The House Manager lives on-site. All residents are required to sign a lease, and are subject to House Rules including curfew, work requirements, house meetings, and maintaining a substance and tobacco free environment.

• Senior/disabled assisted low-income housing at Winchester Senior Apartments, Elko. Income, age, and disability restrictions apply.
• Affordable housing consists of single bedroom units at High Desert Apartments. Income restrictions apply.
• Continuum of Care Apartments are available in Elko. They are single bedroom units with some furnishings included. These apartments are designated as permanent housing for homeless individuals. Some qualifications and restrictions apply. These are assisted low-income units.
